Let’s get started!

Your first visit will include filling out a general health questionnaire and a form specific to your current complaint(s). You can read more about the physical itself in the services section of this website. In order to formulate a proper diagnosis, please bring any pertinent medical records from any other health care providers. X-rays may be required for some cases. I do not take x-rays on site, but I will help you set up an appointment with a nearby and inexpensive outpatient facility if and when necessary. Once all of the information from the physical, any possible x-rays, and your medical records have been reviewed, a diagnosis and a care plan will be created especially for you. 

For a shorter time in the lobby, please make sure that you complete your new patient intake form before you arrive. It will be included as a link in your welcome email once you schedule an appointment.

I understand that affordable health care is important, and I have several payment plans that will reduce the cost of regular visits. That said, it is important to note that I am not currently taking insurance. However, it may be possible that I will be able to provide you with a "super bill," that you can turn in for reimbursement to your insurance company. If you have any questions, please don't hesitate to call and see what I can arrange for you.
